Composition:
Each contains: Isotretinoin BP (Mfg. Specs, BP) 40mg Clinical Particulars
Therapeutic Indications:
A treats severe forms of acne (nodular or conglobate acne, or acne prone to permanent scarring) and acne that hasn’t responded to standard therapies involving systemic antibacterial and topical therapy.
Dosage and Method of Administration:
Physicians experienced in systemic retinoid use and knowledgeable about its teratogenic risks should exclusively prescribe Arynoin. Therapy should begin at 0.5 mg/kg daily, with most patients requiring 0.51.0 mg/kg daily. Severe cases may require up to 2.0 mg/kg daily. Take capsules with food once or twice daily. A cumulative dose of 120150 mg/kg per treatment is common, with a duration of 1624 weeks. Remission often occurs after one course. In case of relapse, another course may be given with the same dose
Special Dosage Instructions
Patients with Renal Impairment:
Start at a lower dose (e.g., 10 mg/day) and increase up to 1 mg/kg/day or the maximum tolerated dose
Contraindications:
- Women in childbearing potential
- Pregnant or breastfeeding women
- Patients receiving tetracyclines
- Hepatic insufficiency
- Preexisting hypervitaminosis A
- Elevated blood lipid values
- Hypersensitivity to isotretinoin
Arynoin Is Teratogenic:
Pregnancy must be avoided. Female patients must:
- Use effective contraception for 1 month before, during, and after treatment
- Have a negative pregnancy test
- Start therapy on the 2nd or 3rd day of their menstrual period
- Understand the risk and need for rigorous follow-up
Psychiatric Disorders:
- Reports have indicated depression, psychosis, and rare suicide attempts.
Skin and Subcutaneous Tissue Disorders:
Initial acne exacerbation may occur but usually subsides within 710 days. Sun exposure should be avoided
Eye Disorders:
Dry eyes, corneal issues, decreased night vision, and others may occur but often resolve after treatment discontinuation
Musculoskeletal and Connective Tissue Disorders:
Myalgia, arthralgia, and increased serum creatine phosphokinase can occur
Benign Intracranial Hypertension:
Rare cases have been reported
Hepatobiliary Disorders:
Monitor liver function or enzymes and note transient increases.
Renal Insufficiency:
Arynoin can be given to patients with renal insufficiency
Lipid Metabolism:
Monitoring serum lipids is necessary, and elevated triglyceride levels (>800 mg/mL) can associate with acute pancreatitis.
