Topagen 25mg Tablets contain Topiramate, a broad-spectrum anticonvulsant with multiple complementary mechanisms of action: voltage-dependent sodium channel blockade, enhancement of GABA-mediated chloride influx, antagonism of AMPA/kainate glutamate receptors, and inhibition of carbonic anhydrase (isoforms II and IV). This multi-modal activity makes Topiramate effective across a range of epilepsy types and in migraine prophylaxis.

The 25mg tablet is typically used as the starting and titration dose — Topiramate is always initiated at a low dose and increased slowly (25mg increments weekly) to minimise cognitive side effects.

Approved Indications

• Epilepsy — adjunctive or monotherapy for partial-onset seizures, primary generalised tonic-clonic seizures, and seizures associated with Lennox-Gastaut syndrome (adults and paediatric patients ≥2 years)

• KIDNEY STONES (NEPHROLITHIASIS): Carbonic anhydrase inhibition reduces urinary citrate and may promote calcium phosphate stone formation. Drink adequate fluids (2–3 litres/day). Avoid high-protein diets.
• METABOLIC ACIDOSIS: Topiramate inhibits carbonic anhydrase, which can reduce serum bicarbonate. Monitor bicarbonate levels, especially in paediatric patients and those on ketogenic diets.
• ACUTE ANGLE-CLOSURE GLAUCOMA: A rare but serious early-onset adverse effect. Stop Topiramate immediately and seek urgent eye assessment if you develop sudden eye pain, redness, or visual changes.
• WEIGHT LOSS: Anorexia and weight reduction are common — monitor weight, especially in paediatric patients.

Directions for Use

Always follow your prescriber's dosing schedule. Topiramate is titrated slowly — typically starting at 25mg/day and increasing by 25mg weekly to minimise side effects. Tablets may be swallowed whole or carefully split. Do not stop abruptly — withdrawal can precipitate seizures. Maintain adequate hydration throughout treatment.
