Minoxidil is the only FDA-approved topical treatment for androgenetic alopecia, backed by more than 40 years of research. Originally a blood-pressure drug, its hair-growth effect was discovered by chance and later developed into the topical treatments used worldwide today. This guide is part of our Hair Fall Solutions series.
How Minoxidil Works
Minoxidil widens the blood vessels around hair follicles, increasing the delivery of oxygen and nutrients. It also extends the anagen (growth) phase while shortening the resting phase, producing thicker, longer hairs, and acts as a mild anti-androgen that reduces follicle sensitivity to DHT. This is why it often succeeds where nutrition alone does not, particularly in genetically predisposed people.
Formulations Available in Pakistan
The 5% strength is the standard for men. Follinox 5% solution and Hair Max Plus 5% spray are popular pharmaceutical-grade options. A gentler 2% strength suits women and sensitive scalps. Combination sprays such as 5% minoxidil with 0.1% finasteride and Healers Pharma 2% minoxidil with finasteride add DHT inhibition for stronger results. Low-dose oral minoxidil is another option for extensive loss, but needs medical supervision and cardiovascular monitoring.
What Results to Expect
Most consistent users see shedding stabilise, and many see measurable regrowth within six months. A typical timeline:
Timeline
Months 1 to 2: an initial shedding phase as weak hairs are replaced. Months 3 to 4: shedding slows and new growth becomes visible. Months 6 to 12: peak density improvement. Beyond 12 months: a maintenance phase requiring continued use.
Women often respond particularly well to minoxidil, but should start with the 2% strength to minimise the small risk of unwanted facial hair from accidental contact.
How to Apply
Apply to a clean, dry scalp, targeting thinning areas rather than the hair itself. Use the measured dose (about 1 ml of solution or half a capful of foam) twice daily, spread gently with the fingertips, and allow around four hours before washing. Wash your hands afterwards. Minoxidil pairs well with ketoconazole shampoo used two to three times weekly. Microneedling (0.5 mm) once weekly may improve absorption; apply minoxidil at least 24 hours afterward to avoid irritation.
Side Effects
Most side effects are mild and settle within two to three weeks as the scalp adapts. They include redness, itching or flaking at the application site (switching to a foam can help); a temporary increase in shedding during weeks 2 to 6, which signals weak hairs cycling out; and fine hair growth on the face or hands from accidental contact, which reverses with careful application. Serious reactions are rare: anyone with a heart condition should consult a doctor first, and true allergy (severe rash or swelling) needs immediate discontinuation.
A Long-Term Commitment
Minoxidil manages hair loss rather than curing it: stopping treatment leads to a gradual return to baseline within six to twelve months. Think of daily application as a routine like brushing your teeth. For underlying hormonal causes, minoxidil helps symptoms while the root cause is treated, and it combines well with advanced clinical treatments and hair-growth supplements.
