Description
Pequit XR 150mg Tablets — Quetiapine Fumarate Extended Release
Pequit XR 150mg Tablets contain Quetiapine Fumarate in an extended-release (XR) formulation. Unlike the immediate-release form, XR delivers quetiapine over a prolonged period — resulting in smoother plasma concentration curves, reduced peak-related sedation, and the convenience of once-daily dosing. This improved tolerability profile can support better long-term adherence.
Approved Clinical Indications
- Schizophrenia — maintenance and acute treatment with once-daily dosing
- Bipolar I Disorder — acute mania and mixed episodes
- Bipolar Depression — depressive episodes
- Adjunct MDD — add-on to antidepressant in treatment-resistant depression
XR vs IR — Key Clinical Differences
- Dosing: Once daily (XR) vs. twice or three times daily (IR)
- Peak sedation: Reduced with XR due to lower Cmax
- Adherence: Simplified once-daily schedule may improve compliance
- Do not crush/chew: Swallow whole to preserve extended-release mechanism
Important Clinical Notes & Monitoring
- Metabolic monitoring: Weight, glucose, HbA1c, and lipids — monitor at baseline and every 3–6 months.
- QTc prolongation: ECG monitoring at baseline and with cardiac risk factors.
- Cataract monitoring: Lens examination recommended for long-term use.
- Tardive dyskinesia: Assess periodically with long-term use.

Usage & Benefits
- Once-daily convenience: XR formulation enables simplified once-daily dosing — improves adherence vs. multiple-dose IR regimens
- Smoother plasma levels: Reduced Cmax minimises peak-driven sedation and side effects
- Schizophrenia maintenance: Sustained D2 and 5-HT2A blockade throughout the day
- Bipolar coverage: Effective for both acute mania and bipolar depressive episodes
- Improved tolerability profile: XR may offer better tolerability for patients who experienced sedation on IR formulation
How To Use
Take exactly as prescribed by your psychiatrist — once daily, typically in the evening. Swallow the tablet whole with water; do not crush, split, or chew (this destroys the extended-release mechanism and delivers the full dose at once). Avoid alcohol and other CNS depressants. Do not stop suddenly. Monitor weight, blood glucose, and lipids at baseline and regularly. Report any muscle stiffness, unusual movements, or vision changes to your prescriber.
FAQs
What is the difference between Pequit XR and Pequit (IR)?
Pequit XR releases quetiapine slowly over 24 hours (extended-release), allowing once-daily dosing with lower peak drug levels and reduced sedation. Pequit IR requires multiple daily doses and produces higher peak levels.
Can I crush or break the Pequit XR tablet?
No. Crushing or splitting destroys the extended-release coating and delivers the full dose immediately — which is dangerous. Swallow whole.
What is the standard maintenance dose of Quetiapine XR for schizophrenia?
Typically 400–800mg once daily. The 150mg dose is used during titration or for lower-dose indications. Your prescriber determines the appropriate dose.
Does Pequit XR still require metabolic monitoring?
Yes. Metabolic risk (weight gain, glucose dysregulation, dyslipidaemia) applies to all quetiapine formulations regardless of release type.
