Revalp 250mg Tablets — Divalproex Sodium
Revalp 250mg Tablets contain Divalproex Sodium, a stabilised formulation combining sodium valproate and valproic acid in an enteric-coated form that is better tolerated gastrointestinally than plain valproic acid. Divalproex is an established anticonvulsant, mood stabiliser, and migraine prophylactic agent with multiple mechanisms of action: enhancement of GABA activity, reduction of glutamate excitation, sodium channel stabilisation, and histone deacetylase (HDAC) inhibition.

- Epilepsy — partial (focal) seizures, generalised tonic-clonic seizures, absence seizures, myoclonic seizures

CRITICAL SAFETY WARNINGS — Read Before Prescribing or Dispensing
- MAJOR TERATOGEN — Valproate in Pregnancy: Risk of neural tube defects (spina bifida) is up to 10 times the background population rate. Risk of neonatal valproate syndrome (craniofacial abnormalities, limb defects, autism spectrum effects). Valproate is contraindicated in pregnancy unless no safer alternative exists. Women of childbearing potential must use effective contraception and receive specialist counselling. REMS-type programmes apply in multiple countries.
- Hepatotoxicity: Fatal hepatic failure has occurred, primarily in children under 2 years on polytherapy. Monitor liver function tests (LFTs) at baseline and frequently in the first 6 months. Stop immediately if LFTs rise significantly or symptoms of liver failure develop.
- Pancreatitis: Life-threatening pancreatitis has been reported at all ages. Discontinue immediately if abdominal pain develops and check serum lipase/amylase.
- Thrombocytopenia: Valproate can reduce platelet counts — monitor full blood count (CBC) periodically.
Important Drug Interactions
- Carbapenem antibiotics (meropenem, imipenem, ertapenem) dramatically reduce valproate plasma levels — may cause seizure breakthrough. Avoid combination if possible.
- Lamotrigine: Valproate inhibits lamotrigine metabolism — lamotrigine dose must be halved when added to valproate therapy.
- Phenytoin, carbamazepine, rifampicin: Enzyme inducers that reduce valproate levels.
- Aspirin (in children): Displaces valproate from protein binding — avoid high-dose aspirin combinations.
