Saveril 100mg Tablets — Clozapine for Treatment-Resistant Schizophrenia
Prescription required. This is a specialist medication dispensed only against a valid prescription with current blood monitoring results.
Saveril 100mg Tablets contain Clozapine, an atypical antipsychotic indicated exclusively for treatment-resistant schizophrenia — defined as schizophrenia that has not responded adequately to at least two different antipsychotic medications given at adequate doses for adequate duration. Clozapine is also indicated for reducing recurrent suicidal behaviour in patients with schizophrenia or schizoaffective disorder.
Clozapine acts through broad receptor antagonism including dopamine D1–D4, serotonin 5-HT2A, histamine H1, muscarinic M1–M5, and alpha-1 adrenergic receptors. This unique pharmacological profile explains both its superior efficacy in refractory cases and its complex adverse-effect burden.
CRITICAL SAFETY WARNINGS
- AGRANULOCYTOSIS (potentially fatal): Clozapine causes severe neutropenia/agranulocytosis in approximately 1% of patients. Mandatory WBC and ANC (absolute neutrophil count) monitoring is required before initiation and throughout treatment — typically weekly for the first 6 months, then fortnightly, then monthly. Clozapine cannot be legally dispensed without current blood test documentation. Immediately contact your prescriber if you develop fever, sore throat, or signs of infection.
- MYOCARDITIS AND CARDIOMYOPATHY: Potentially fatal cardiac inflammation — most commonly in the first 6–8 weeks of treatment. Report any chest pain, palpitations, shortness of breath, or fever to your doctor immediately. Baseline ECG and cardiac monitoring are required.
- SEIZURES: Dose-dependent risk — highest at doses above 600mg/day. Avoid activities requiring alertness (driving, operating machinery) until seizure risk is established.
- SEVERE CONSTIPATION: Clozapine's strong anticholinergic effects can cause life-threatening bowel obstruction. Active bowel monitoring and prophylactic laxative regimens are routinely required.
- METABOLIC SYNDROME: Significant risk of weight gain, hyperglycaemia, and dyslipidaemia — monitor fasting glucose, lipid panel, and weight regularly.
- HYPERSALIVATION: Drooling is common, especially during sleep. May be managed with dose adjustment or adjunct medication.
- SMOKING INTERACTION: Smoking induces CYP1A2 enzymes that metabolise clozapine. Patients who stop smoking will have significantly higher clozapine plasma levels — a potentially toxic rise. Inform your prescriber immediately if you change smoking status.
Approved Indications
- Treatment-resistant schizophrenia (failed ≥2 adequate antipsychotic trials)
- Reduction of suicidal behaviour in schizophrenia or schizoaffective disorder
Directions for Use
Use only under the supervision of a psychiatrist or specialist physician. Dose is titrated slowly from a low starting dose. Do not stop or adjust your dose without medical guidance — abrupt discontinuation can cause cholinergic rebound and rapid psychotic relapse. Blood tests must be current and on file before each dispensing.

FAQs
-
Q: Why is blood monitoring required with Saveril?
A: Clozapine carries a risk of agranulocytosis (dangerous reduction in white blood cells). Mandatory WBC/ANC monitoring detects this early before it becomes life-threatening. No blood results = no dispensing. -
Q: Can Saveril be stopped suddenly?
A: No. Abrupt discontinuation risks rapid psychotic relapse and cholinergic rebound (nausea, vomiting, excessive sweating). Any dose reduction must be gradual and supervised by your psychiatrist. -
Q: What should I do if I get a fever while on Saveril?
A: Contact your prescriber immediately and present for a WBC/ANC blood test. Fever can be an early sign of agranulocytosis, which is a medical emergency. -
Q: Can Saveril be used for other psychiatric conditions?
A: Clozapine is approved specifically for treatment-resistant schizophrenia and reducing suicidal behaviour in schizophrenia/schizoaffective disorder. Any other use is off-label and requires specialist judgement.
Usage & Benefits
- Gold-standard for treatment-resistant schizophrenia unresponsive to other antipsychotics
- Superior efficacy for positive symptoms: hallucinations, delusions, thought disorganisation
- Approved for reducing suicidal behaviour in schizophrenia/schizoaffective disorder
- Broad receptor antagonism provides antipsychotic, anxiolytic, and mood-stabilising effects
- Evidence-based choice when standard antipsychotics fail after adequate trials
How To Use
Saveril must only be used under the direct supervision of a psychiatrist or specialist physician. Dose is started low and titrated slowly to minimise risk. WBC and ANC blood tests must be performed before initiation and at mandatory intervals throughout treatment — dispensing is not possible without current test results on file. Do not stop or adjust the dose without medical guidance. Abrupt discontinuation can cause cholinergic rebound and rapid psychotic relapse. Report any fever, sore throat, mouth ulcers, or signs of infection immediately to your prescriber.
FAQs
Why is blood monitoring required with Saveril?
Clozapine causes agranulocytosis (dangerous white blood cell reduction) in ~1% of patients. Mandatory WBC/ANC monitoring detects this before it becomes life-threatening. No current blood results = no dispensing.
Can Saveril be stopped suddenly?
No. Abrupt discontinuation risks rapid psychotic relapse and cholinergic rebound (nausea, sweating, vomiting). Any dose reduction must be gradual under specialist supervision.
What happens if I get a fever while on Saveril?
Contact your prescriber immediately and get a WBC/ANC blood test the same day. Fever can be an early sign of agranulocytosis — a medical emergency.
What is the significance of the smoking interaction?
Smoking induces CYP1A2 enzymes that metabolise clozapine. Stopping smoking significantly raises clozapine blood levels, risking toxicity. Inform your prescriber immediately if your smoking status changes.
