Topagen 50mg Tablets contain Topiramate 50mg, a broad-spectrum anticonvulsant used at maintenance and therapeutic doses for epilepsy and as a proven prophylactic agent for migraine prevention. Topiramate works through multiple complementary mechanisms: it blocks voltage-gated sodium channels to inhibit neuronal firing, potentiates GABA activity to enhance inhibitory neurotransmission, antagonises AMPA/kainate glutamate receptors to reduce excitatory drive, and inhibits carbonic anhydrase enzymes. Together these actions reduce the frequency and severity of seizures and prevent migraine episodes.

The 50mg tablet represents a standard maintenance dose in the titration schedule — most patients are escalated to this level after tolerating the 25mg starting dose over 1–2 weeks.

Approved Indications

• Epilepsy — adjunctive and monotherapy for partial-onset seizures, primary generalised tonic-clonic seizures, and seizures in Lennox-Gastaut syndrome (adults and children ≥2 years)

IMPORTANT SAFETY WARNINGS

• TERATOGENICITY — PREGNANCY RISK: Topiramate significantly increases the risk of oral cleft defects (cleft lip/palate) in infants exposed during pregnancy. Female patients of childbearing potential must use reliable contraception throughout treatment. Any pregnancy must be immediately reported to the prescribing specialist.
• COGNITIVE EFFECTS ("Dopamax"): Word-finding difficulty, cognitive slowing, and memory problems are common and well-documented. Gradual titration reduces severity. Significant cognitive impairment warrants a prescriber review of dose or agent.
• KIDNEY STONES: Carbonic anhydrase inhibition reduces urinary citrate, increasing the risk of calcium phosphate nephrolithiasis. Maintain high fluid intake (2–3 litres/day). Avoid high-protein or ketogenic diets without medical guidance.
• METABOLIC ACIDOSIS: Reduced serum bicarbonate can occur — monitor levels, especially in children and patients on concurrent carbonic anhydrase inhibitors.
• ACUTE ANGLE-CLOSURE GLAUCOMA: Rare but serious. Stop immediately and seek urgent ophthalmology assessment if sudden eye pain, blurred vision, or redness develops.
• WEIGHT LOSS: Common; may be clinically significant in underweight or paediatric patients. Monitor regularly.

Directions for Use

Take exactly as prescribed. Topiramate is always titrated gradually (25mg weekly increments) to the target dose — do not increase dose faster than instructed. Take with or without food. Swallow whole or carefully split. Maintain adequate hydration. Do not discontinue abruptly — sudden withdrawal may trigger seizure escalation.
