What is Coversam 4mg/10mg?
Coversam 4mg/10mg Tablets are a fixed-dose combination of Perindopril 4mg (ACE inhibitor) and Amlodipine 10mg (calcium channel blocker, dihydropyridine class). This dual-mechanism antihypertensive is indicated for the treatment of hypertension in adults whose blood pressure is not adequately controlled on either agent alone, or as initial therapy where combination treatment is clinically appropriate.
How Does Coversam 4mg/10mg Work?
Perindopril inhibits angiotensin-converting enzyme (ACE), reducing angiotensin II formation and aldosterone secretion, which lowers peripheral vascular resistance and blood pressure. Amlodipine blocks voltage-gated L-type calcium channels in vascular smooth muscle and cardiac tissue, causing vasodilation and reducing cardiac afterload. Together, these complementary mechanisms achieve blood pressure reduction with reduced fluid retention risk compared to CCBs alone.
Uses of Coversam 4mg/10mg
Indicated for: hypertension in adults requiring combination ACE inhibitor and calcium channel blocker therapy; stable coronary artery disease in hypertensive patients. For patients managed on the lower amlodipine dose, Coversam 4mg/5mg is also available.
Dosage & Administration
One tablet (4mg/10mg) once daily, preferably in the morning before a meal or with food. Do not crush or split. Dose adjustment required in renal impairment (CrCl 30–60 mL/min: consider lower dose). Not recommended in severe renal impairment or anuria. Take at the same time each day for consistent blood pressure control.
Side Effects
Common (ACE inhibitor component): dry persistent cough (class effect, may require switching), dizziness, hyperkalaemia. Common (amlodipine component): peripheral oedema (ankle swelling), flushing, headache, palpitations. Rare but serious: angioedema (swelling of face, lips, tongue — discontinue immediately), hypotension (especially first dose or with dehydration).
Precautions
Contraindicated in history of ACE inhibitor-induced angioedema, concurrent sacubitril/valsartan use, pregnancy (teratogenic), and bilateral renal artery stenosis.
