Brivace 25mg Tablets – Brivaracetam for Epilepsy & Seizure Control
Description
What is Brivace 25mg?
Brivace 25mg Tablets contain Brivaracetam 25mg — a third-generation antiepileptic drug (AED) manufactured by Hilton Pharma. The 25mg strength is used as a starting or low-maintenance dose for adults initiating treatment, patients with hepatic impairment, or those being titrated cautiously. It is prescribed for adjunct treatment of partial-onset (focal) seizures in adults and adolescents aged ≥16 years. This is a prescription-only (Rx) medicine.
How Does Brivace Work?
Brivaracetam is a selective, high-affinity ligand for synaptic vesicle protein 2A (SV2A) — a glycoprotein integral to neurotransmitter vesicle exocytosis. By modulating SV2A, Brivaracetam reduces aberrant neuronal burst-firing that underlies focal seizures without significantly suppressing normal brain activity. Its high SV2A affinity (~15–30× greater than levetiracetam) translates into potent anticonvulsant effect at lower doses, and its superior CNS penetration allows rapid onset of action.
Uses of Brivace 25mg
Brivace 25mg is used for: initiation of Brivaracetam therapy (titration phase), dose reduction in hepatic impairment, patients sensitive to higher doses who need gradual up-titration, and adjunct treatment of partial-onset seizures in patients ≥16 years not adequately controlled on other AEDs.
Dosage & Administration
Usual starting dose: 25–50mg twice daily. Doses can be up-titrated to 50mg twice daily (100mg/day standard) or 100mg twice daily (200mg/day maximum) based on seizure control and tolerability, under neurologist supervision. Take with or without food. Swallow whole — do not crush or chew. Never stop abruptly — always taper under medical supervision.
Side Effects
Common: dizziness, somnolence, fatigue, nausea. Compared to levetiracetam, Brivaracetam tends to have fewer neuropsychiatric side effects (irritability, aggression), making it a preferred option in some patients. Monitor for mood changes, depression, and suicidal ideation — a class-wide risk for all AEDs. Report any significant behavioural changes promptly.
Precautions
Do not stop Brivace 25mg suddenly — abrupt withdrawal can provoke breakthrough seizures requiring emergency treatment. Dose adjustment required in moderate-to-severe hepatic impairment. Not recommended for use in children under 16 years for this indication. Do not drive or operate heavy machinery until you know how the medication affects you and seizure control is stable. This is an Rx medicine — use only under neurological supervision.
Frequently Asked Questions
Q: Why start at 25mg rather than 50mg?
A: The 25mg dose allows a cautious titration start, particularly important for patients with hepatic impairment, those sensitive to CNS medications, or when minimising side effects during initiation. Dose can be stepped up to Brivace 50mg as tolerated.
Q: Can Brivace 25mg be used alone for epilepsy?
A: Brivaracetam is primarily licensed as adjunct therapy for focal seizures. Discuss with your neurologist whether monotherapy is appropriate for your seizure type.
Q: Where can I find the full Brivace dose range in Pakistan?
A: All three doses are available at Derma.pk: Brivace 25mg, Brivace 50mg, and Brivace 100mg. Browse our Anti-Epileptic collection.
